HONOLULU (AP) — The fate of gay marriage in Hawaii is in the hands of the state House after smoothly passing in the Senate.<br />
It's already been a bumpier ride for the legislation in the House.<br />
A joint <a href="http://www.sfgate.com/?controllerName=search&action=search&channel=news&search=1&inlineLink=1&query=%22House+committee%22">House committee</a>
involving about half the state's representatives is scheduled for
Thursday, with plans to go at least 14 hours with breaks and possibly
extending to Friday if enough people want to testify. Officials say
about 3,000 people had submitted written testimony by midday Wednesday.<br />
The
House referred the bill to a joint committee Wednesday night, but not
before a nearly four-hour session that was riddled with arguments,
recesses, private caucus meetings and maneuvering by lawmakers on both
sides to the issue to either move the measure along, delay it or advance
a rival bill to put the matter to a public vote through a
constitutional amendment.<br />
At one point, Republican Rep. <a href="http://www.sfgate.com/?controllerName=search&action=search&channel=news&search=1&inlineLink=1&query=%22Gene+Ward%22">Gene Ward</a>, who is against legalizing same-sex marriage, argued with Democratic House Speaker <a href="http://www.sfgate.com/?controllerName=search&action=search&channel=news&search=1&inlineLink=1&query=%22Joseph+Souki%22">Joseph Souki</a> over whether he could talk about the bill during its first reading. Souki had said he could not.<br />
"Why are you muzzling us," Ward said.<br />
Souki told Ward he was out of order, then quickly called a recess.<br />
The
bill passed easily, 20-4, earlier Wednesday on the Senate floor with
one senator abstaining because his mother had died in the morning. The <a href="http://www.sfgate.com/?controllerName=search&action=search&channel=news&search=1&inlineLink=1&query=%22Hawaii+Senate%22">Hawaii Senate</a> is dominated by <a href="http://www.sfgate.com/?controllerName=search&action=search&channel=news&search=1&inlineLink=1&query=%22Democrats%22">Democrats</a>, with only one Republican.<br />
Democratic Sen. <a href="http://www.sfgate.com/?controllerName=search&action=search&channel=news&search=1&inlineLink=1&query=%22Clayton+Hee%22">Clayton Hee</a> said before voting for the bill in the Senate that the moment is career-defining and lawmakers should embrace it.<br />
"I
ask you to expand the meaning of the word 'aloha' to truly include
everyone," regardless of race, creed or sexual orientation, Hee said.<br />
House Majority Leader <a href="http://www.sfgate.com/?controllerName=search&action=search&channel=news&search=1&inlineLink=1&query=%22Scott+Saiki%22">Scott Saiki</a>
has said it's likely the chamber will amend the bill to change
religious exemptions. The Senate bill exempts ministers and other clergy
— but not for-profit businesses — from having to perform gay
wedding ceremonies.<br />
"The House committees recognize that there is still a lot of public concern about the scope of the exemptions," Saiki said.<br />
The
House is made up of 44 Democrats and seven Republicans. While Souki has
said he believes there's enough support to pass gay marriage, some
Democrats plan to vote no on the bill.<br />
Hee
said after the Senate vote that he has spoken with leadership in the
House and told them that senators may not support expanded religious
exemptions if they allow gay couples to be discriminated against as a
separate class of people from others.<br />
"Those are efforts that I would caution against," Hee told reporters after the Senate vote.<br />
Two lawmakers, Sens. Sam Slom and <a href="http://www.sfgate.com/?controllerName=search&action=search&channel=news&search=1&inlineLink=1&query=%22Mike+Gabbard%22">Mike Gabbard</a>,
spoke in opposition to the bill and criticized everything from the
special session itself to the consequence they say gay marriage will

Bitcoins, now valued at over $200 a piece, were seen as the currency of the black market, back when the website <a href="http://abcnews.go.com/US/silk-road-website-dealt-drugs-guns-assassins-bitcoins/story?id=20446005" target="internal">Silk Road</a> was still up and running.

But there are many people who want to cast off its shady past and make
it a legitimate currency. Robocoin, a company based in Nevada, is
looking to make that happen by installing the first bitcoin ATMs on
Tuesday at a Waves Coffee House in Vancouver, Canada.

Because the bitcoin currency itself is distributed among all of its
users, there is no centralized bitcoin bank. Instead, people buy and
sell bitcoins by visiting websites, finding a user willing to sell them
some bitcoins, and then <a href="https://en.bitcoin.it/wiki/Buying_Bitcoins_%28the_newbie_version%29" target="external">directly wiring them money</a>.
But new buyers are left waiting until their money transfer is cleared, a
couple of days, before they actually see the bitcoins appear in their
wallet. The same applies to sellers looking to cash out, but having to
wait for their payment to arrive.

Jordan Kelley, CEO of Robocoin, said that the ATMs are making those
money transfers much quicker. "People need to be able to get some of
their money out," he told ABC News. "Our customers can walk up to a
machine and buy and sell bitcoins for cash."

The Robocoin kiosk first verifies customers' identities by scanning
their palm prints. Customers can then feed cash into the machine to
deposit as bitcoins, or withdraw bitcoins from their electronic wallet
and get physical cash. Instead of waiting around a couple of days, the
process only takes a couple of minutes.

Though Robocoin makes the machinery responsible for doling out cash,
it's up to bitcoin enthusiasts to actually make them available to the
public. Mitch Demeter, a bitcoin broker who co-founded Bitcoiniacs, said
that he was impressed with Robocoin's machines. "The ATMs are basically
our store in a box," he said. "We bought five in total and we're
spreading them across Canada."

but the government itself is hesitant to welcome them into the American
marketplace. "The United States requires more licensing to operate,"

Jonas Brothers fans hoping the band would stay together just a little
bit longer were splashed with cold reality on Tuesday, as the <a href="http://www.people.com/people/article/0,,20750228,00.html" target="_hplink">siblings announced to People that they had split up</a>.<br />
"It's over for now," Kevin Jonas told the tabloid. (<a href="http://www.people.com/people/article/0,,20750228,00.html" target="_hplink">Nick and Joe Jonas</a> also made similar statements to People.)<br />
That announcement has been a long time coming for the Jonas Brothers.
At the beginning of October, two days before the JoBros were planning
to head out on a fall tour, <a href="http://www.people.com/people/article/0,,20743699,00.html" target="_hplink">People reported that all 19 of their concerts had been canceled.</a><br />
"There is a deep rift within the band," Jesse Derris, a
representative for the Jonas Brothers, told People. "There was a big a

Magic Johnson is bracing himself for a rough season. He might be part owner of the <a href="http://espn.go.com/mlb/team/_/name/lad/los-angeles-dodgers">Los Angeles Dodgers</a>,
who just went to the National League Championship Series, but he'll
always be a Lakers fan, and their prospects don't look as rosy.<br />
"Laker fans are spoiled. I don't know how they're going to react now,
because this is going to be a tough season," Johnson said Tuesday
afternoon during an hourlong interview on the "Max and Marcellus Show"
on ESPNLA 710 radio. "This is going to be one of the roughest seasons
that the Lakers have ever faced. You're waiting on <a href="http://espn.go.com/nba/player/_/id/110/kobe-bryant">Kobe [Bryant]</a> to see if he can get healthy. Nobody knows if he can be what he was. Then you've got <a href="http://espn.go.com/nba/player/_/id/996/pau-gasol">[Pau] Gasol</a> -- who I think is a premier big man -- but he plays better when he has talent around him.<br />
<br />
"You've got guys who are all on one-year contracts, and all were
journeymen on other teams. Laker fans are not used to that. I don't know
if they're going to buy into that. We're going to see. If you want the
Laker fans to really buy in and be involved, you're going to have to win
right away. I don't see that happening. The West is tough this year."<br />
What does Johnson see?<br />
"I'm hoping that they at least make the playoffs," Johnson said. "But
that's going to be tough, especially without Kobe. We have to see what
Kobe we're going to get. Is he going to be that same Kobe or is he going
to be hurt a little bit?"<br />
Johnson said he thought Bryant could still come back from his
ruptured Achilles tendon and be a "dominant player" but might need to
"give up some of those shots" to his teammates to get them going.<br />
"Kobe can get 30 or 40," Johnson said. "But you have to remember, you
need at least three guys who can score. Right now I'm looking at that
roster and going, 'Who else is going to get you 15-20 a night besides
Gasol and Kobe?' "<br />
When asked if he would be willing to be part of the Lakers'
recruiting pitch to potential free agents next summer, Johnson said he
would always help the Lakers if they should ever call upon him. Johnson
sold his ownership stake in the Lakers in 2010 and is no longer
officially affiliated with the team.<br />
"This summer is going to be a crucial summer for the Lakers," Johnson
said. "They have all that cap space. Hopefully [Lakers executive Jim
Buss] has a plan. His dad [the late Dr. Jerry Buss] would've had a plan
two years ago."<br />

The NBA season gets started Tuesday night with a trio of games
highlighted by a clash between the Derrick Rose-led Chicago Bulls and
the two-time defending champion Miami Heat. Las Vegas has the Heat
favored to win a third straight title with the Oklahoma City Thunder,
Los Angeles Clippers and Bulls among the next set of favorites. The NBA
has been on a roll in recent years thanks to a bevy of stars, success by
big market franchises and the international appeal of the game. Total
revenues for the league were expected to approach a <a href="http://www.nba.com/2012/news/11/13/stern-nba-revenue.ap/">record $5 billion last season</a>. Here are the business stories we are tracking this season.<br />
<strong>LeBron James</strong><br />
The four-time MVP is story 1a, 1b and 1c in today’s NBA. We have not seen the focus on one player in the NBA since <a href="http://www.forbes.com/profile/michael-jordan/">Michael Jordan</a>
hung up his high tops for a second time in 1998 after his second
three-peat with the Bulls. ESPN The Magazine devoted its entire NBA
preview issue to all-things LeBron and it is hard to blame them. The
immediate questions are whether James can lead the Heat to another title
and win a fifth MVP (the record is six by Kareem Abdul-Jabbar, while
Jordan and Bill Russell each won five).<br />
The bigger long-term question is where James will be playing next
year. He is expected to exercise his early termination option in his
contract to make him a free agent in the summer of 2014, setting up The
Decision 2.0. Three years after James’ departure from Cleveland, the
franchise value of the Cavaliers is down 9% and the value of the Heat is
up 71%. James said he will not address his free agency during the
season, but the Heat, followed by the Cavs are expected to be the
leaders in the race for his services.<br />
<strong>TV Contract</strong><br />
The NBA’s current contract with partners ABC/ESPN and TNT runs
through the 2015-2016 season and is worth $930 million a year on
average, but negotiations on an extension are expected to be completed
over the next 12 months. Look for the NBA to double its annual take with
the craving for sports programming rights at an all-time high. The
addition of NBC Sports Network and Fox Sports 1 as national entities
bumps up the number of players bidding on the rights packages. TV
ratings were down last season, but they hit 10-year highs the previous
year. The 10.4 rating for the 2013 NBA Finals was the second highest in
the past decade.<br />
<strong>David Stern’s Retirement</strong><br />
Commissioner Stern will retire in February after 30 years in the
NBA’s top spot. The league has exploded under his watch. When he took
over, CBS was paying $22 million a year for broadcast rights to games
and the playoffs (outside of the Finals) were shown on tape-delay. NBA
teams were worth about $400 million collectively in 1984 and now are <a href="http://www.forbes.com/sites/kurtbadenhausen/2013/01/23/billion-dollar-knicks-and-lakers-top-list-of-nbas-most-valuable-teams/">worth more than $15 billion.</a> Long-time
deputy Adam Silver will take the reins, and he is expected to be the
driving force in the next round of TV deals. Silver has been an integral
part of just about every major NBA decision in recent years, including
the settlement of the 2011 lockout.<br />

For $20, Uber will deliver a kitten for you to snuggle for fifteen

Authorities in South Carolina say that six people, including two
children, have been killed in a shooting at a home in the town of
Greenwood.<br />
Greenwood County Sheriff Tony Davis said that deputies received a
call at around 6 p.m. Tuesday from a man inside the home who said he was
intent on "hurting himself," according to FoxCarolina.com. Sheriff's
Department spokesman John Long said that while deputies were on their
way to the scene, they received a 911 call from a neighbor saying two
more children had arrived at her home and told her shots had been fired
in the house.<br />
Davis said that the shooter was among the dead and there was no further danger to the community.<br />
"I've been in law enforcement in Greenwood since 1975 and to my
knowledge this is the first [situation] that I responded to with this
many deceased," Davis said.<br />
Long said that the shooting appeared to stem from a domestic dispute.
The bodies were discovered by a SWAT team that entered the house after
failing to make contact with anyone inside.<br />
Long told the Associated Press that the ages of the dead ranged from 9
years old to early 50s. Their identities were not immediately
released.<br />
Greenwood, a town of approximately 70,000 people, is located 80 miles west of South Carolina's capital, Columbia.<br />

WASHINGTON (AP) — A bipartisan group of lawmakers is
pressing for a four-year delay to changes to the federal government's
flood insurance program that are threatening to sock thousands of people
with unaffordable premium hikes.</div>
The move comes as the government
is beginning to implement a significant overhaul of the much-criticized
program. That overhaul passed last year with sweeping support. The
revamped program was backed by both liberals and tea party conservatives
but has caused a panic in places like Staten Island, N.Y., and the New
Jersey coast and in flood-prone areas of Louisiana, Mississippi and
Florida, where higher rates threaten to push some people out of their
homes.<br />
Some of the most ardent supporters of delaying the premium
increases are conservative Republicans from Southern states, where the
new rules have sent some home values plummeting because of uncertainty
over insurance rates and because subsidized rates can't be passed along
to buyers. New flood maps threaten to saddle some homeowners who are
paying a few hundred dollars a year now with annual premiums of more
than $20,000.<br />

Last year's legislation promises premium increases to 1.1
million homeowners who've received subsidized, below-risk coverage and
could sock even more homeowners whose homes met older building standards
or were deemed at lower risk under previous flood maps. Under the old
rules, they could retain their old rates since they followed the rules
when they bought or built their home, but they will soon lose those
"grandfathered" rates under the new law.<br />
The new legislation, to
be unveiled at a Capitol Hill news conference Tuesday, would delay the
new rates for people purchasing homes from someone who currently has a
subsidized policy or people who face higher rates when flood maps are
updated. People with second homes or whose property has repeatedly been
flooded would still have to pay the higher rates, which are scheduled to
rise by 25 percent a year until their premiums reflect the true risk of
flooding.<br />
Last year's law protected subsidies for people who
receive them if their houses hadn't been recently flooded. The new
legislation would allow them to transfer the subsidy when they sell
their home, thereby propping up home values.<br />

In a US television interview, President
Obama said that national security operations were being reassessed to
make sure the National Security Agency's (NSA) growing technical spying
capability was kept under control.</div>
"We give them policy direction," he told ABC's Fusion network. <br />

Over the last decade, many children who tragically died from flu were
previously healthy kids, without chronic medical conditions, a new
study finds.<br />
Between 2004 and 2012, a total of 830 U.S. children died from flu,
the study found. Of these, more than 40 percent did not have a medical
condition such as asthma or heart disease that would put them at high
risk for <a href="http://www.livescience.com/27312-flu-shot-elderly.html" target="_blank">flu complications</a><img src="http://global.fncstatic.com/static/v/all/img/external-link.png" style="margin: 4px 0px 0px 4px;" />.<br />
What's more, previously healthy children were nearly twice as likely
to die from the flu before they were admitted to the hospital, or within
three days of symptom onset, compared with children who had high-risk

"Healthy kids are at risk, too. I think that's something that a lot
of people don't recognize," said study researcher Dr. Karen Wong, of the
Centers for Disease Control and Prevention (CDC). "People can get very
sick very quickly, even if they don't have an underlying medical
condition."<br />
The findings highlight the importance of <a href="http://www.livescience.com/39765-flu-shot-2013-2014-flu-season.html" target="_blank">flu vaccination</a><img src="http://global.fncstatic.com/static/v/all/img/external-link.png" style="margin: 4px 0px 0px 4px;" />
for all children ages 6 months and older, the researchers said. Of the
511 children who died and who were eligible for vaccination, just 16
percent had received a flu shot.<br />
The findings also suggest that children who develop severe illness
from the flu, who are hospitalized or who are at high risk for
complications (such as those less than 2 years old) should receive
treatment with antiviral drugs as soon as possible, the researchers
said.<br />
The study analyzed information on <a href="http://www.livescience.com/16071-flu-deaths-children.html" target="_blank">child flu deaths</a><img src="http://global.fncstatic.com/static/v/all/img/external-link.png" style="margin: 4px 0px 0px 4px;" />
reported to the CDC. During the study period, the deadliest flu season
for children was 2009 to 2010; over that time, 282 children died. The
mildest flu season was 2011 to 2012, in which 35 deaths occurred.<br />
The majority of deaths, 57 percent, occurred among children with
high-risk medical conditions. Of these, the most commonly reported
condition was a neurological disorder, such as cerebral palsy, with 33
percent of these children having a neurological condition. This was
followed by lung disorders (26 percent), asthma (16 percent) and genetic
disorders (12 percent).<br />
It is especially important that children with high-risk medical
conditions receive flu shots before flu season begins, the researchers
said.<br />
It's not clear why previously healthy children were at greater risk
for death soon after symptom onset. But healthy kids were also more
likely to have a bacterial infection along with their flu infection,
which may have contributed to the differences in outcomes between the
two groups, the researchers said.<br />

viewed by Glenn Greenwald, an American journalist, that were provided
by Edward J. Snowden, the former N.S.A. contractor who has been at the
center of the spying scandal. </div>

According to the Spanish newspapers, the N.S.A. gathered data on phone
numbers and locations but did not monitor the contents of the calls. The
data covered information relating to about 60 million Spanish phone

2010 an alleged foreign intelligence operation involving German

involving Chancellor Merkel," NSA spokeswoman Vanee Vines said.<br />
"News reports claiming otherwise are not true."<br />
The statement does not make it clear whether the president was informed of the bugging operation by other means.<br />
Mr Obama is reported to have told the German chancellor that he knew nothing of the operation when the two leaders spoke.<br />
Germany is sending its top intelligence chiefs to Washington
in the coming week to "push forward" an investigation into the spying
allegations, which have caused outrage in Germany.<br />
Der Spiegel's report, based on leaked documents, says a US
listening unit was based in its Berlin embassy - and similar operations
were replicated in 80 locations around the world.<br />

Brighton, ENGLAND — Southern Britain was battered Monday by a
powerful storm with hurricane-force winds that caused widespread delays
on the nation's roads and trains, and threatened severe disruptions to
airports.<br />
The Met Office said that winds reached 99 mph near the
Isle of Wight at around 6:00 a.m. local time and the Environment Agency
issued dozens of flood alerts for a large swathe of England and Wales.<br />
Exposed
coastlines in Cornwall, Devon, East Sussex, Hampshire and Kent
experienced particularly rough conditions with reports of hundreds of
downed trees. Thousand of homes — as many as 220,000, reports said —
were without power across the southern, coastal parts of the country
Monday.<br />
Police said one teenager was killed after a tree fell on a
home in Kent and a Hertfordshire man in his 50s was killed when a tree
fell on a car, but there were few other reports of injuries. The high
winds and heavy rains forced the cancellation of at least 130 flights at
Heathrow airport and authorities urged the public to take precautions
and "be prepared."<br />
Ahead of the storm's landfall late Sunday night
a 14-year-old boy was feared drowned after being swept out to sea near
the town of Newhaven, in West Sussex.<br />
Local media have dubbed the
storm — one of the most severe to hit the British Isles in decades —
"St. Jude" after the patron saint of lost causes. The saint is

When the FDA <a href="http://www.medpagetoday.com/PainManagement/PainManagement/42493" target="_blank">approved the powerful painkiller Zohydro last week</a>, it did so without requiring the drug to have any built-in biochemical mechanisms aimed at curbing abuse.<br />
That
"omission" makes Zohydro very different from opioids currently on the
market, including OxyContin and Opana, both of which have
abuse-deterrent formulations.<br />
Additionally, the FDA has been
taking steps over the past year to make sure all opioid drugs brought to
the market in the future would incorporate such technologies.<br />
This push follows a major criminal case in which executives of Purdue Pharma, maker of the powerful opioid OxyContin, <a href="http://www.cnbc.com/id/18591525" target="_blank">pleaded guilty to misleading the public about the drug's risk of abuse</a>.<br />
The
company said the drug's extended-release formula would make it harder
to abuse – the same argument initially put forward with the initial
extended release formulation of OxyContin, but crushing OxyContin pills
made them a "drug du jour" for many abusers and forced the company to
add anti-abuse mechanisms.<br />
So why not require extended-release Zohydro to have updated abuse-deterrent properties?<br />
<strong>The Road to Abuse-Deterrent Technology</strong><br />
Pleasure-seekers
have found many ways to get extended-release opioid drugs to release
their full punch all at once, instead of being doled out slowly over
many hours. These pills can be crushed, chewed, or mixed with alcohol or
other liquids to extract the active ingredient.<br />
In response, <a href="http://www.medpagetoday.com/Psychiatry/Addictions/25888" target="_blank">companies have come up with a number of ways to deter that kind of abuse</a>,
such as making the tablet turn chunky instead of powdery when crushed,
or turning it thick and sudsy when mixed with a liquid and drawn into a
syringe.<br />
Some have even combined their opioids with anti-opioid
agents, such as naltrexone, which block opioid receptors in the brain to
send abusers into withdrawal rather than getting them high.<br />
Last January, the <a href="http://www.medpagetoday.com/PublicHealthPolicy/FDAGeneral/36782" target="_blank">FDA released a draft guidance</a>
that would require all new opioid drugmakers to incorporate these
properties into their products. But that document has yet to be
finalized.<br />
<a href="http://sppan.aapainmanage.org/about-us/leadership/" target="_blank">Bob Twillman, PhD</a>,
director of policy and advocacy for the American Academy of Pain
Management, said even if that guidance was in place, it's likely Zohydro
would have been grandfathered because it was "well into the development
process before abuse-deterrent technology became available."<br />
Indeed,
the drug was first developed in 2002 by Elan (which has now merged with
Alkermes), and Zogenix acquired the U.S. rights in November 2007. The
company <a href="http://www.medpagetoday.com/PainManagement/PainManagement/36334" target="_blank">presented Zohydro to the FDA advisory committee</a> just one month before the agency issued its initial draft of the abuse-deterrent guidance.<br />
The issue of abuse-deterrence was a point of contention last spring, when OxyContin maker Purdue Pharma used it as leverage to <a href="http://www.medpagetoday.com/PublicHealthPolicy/PublicHealth/38500" target="_blank">banish generic versions of its drug</a>.<br />
In
2010, four years after it was fined, Purdue upgraded its OxyContin
recipe to include abuse-deterrent mechanisms, and it requested that FDA
block generic versions lacking those properties – which the FDA
ultimately agreed to do, effectively giving Purdue extended patent
protection.<br />
But the agency <a href="http://www.medpagetoday.com/PublicHealthPolicy/PublicHealth/39056" target="_blank">declined to do the same for Opana</a>,
another extended-release opioid. Drugmaker Endo Pharmaceuticals created
an abuse-deterrent version of the original drug in March 2012.<br />
The
FDA said that unlike old OxyContin, the first incarnation of Opana
wasn't pulled from the market for safety reasons, so generic
formulations would be allowed.<br />
"While FDA strongly supports a
transition to abuse-deterrent opioids, we do not believe it is
appropriate or feasible at this time to require all products in the
class to be abuse-deterrent," Morgan Liscinsky, a spokesperson for the
FDA, said in an email to the <em>Journal-Sentinel/MedPage Today</em>.
"Rather, FDA will continue to take a product-by-product approach to
regulatory decisions concerning the abuse-deterrent properties (or lack
thereof) of opioid products."<br />
Liscinsky noted, however, that they will be subject to the <a href="http://www.medpagetoday.com/PublicHealthPolicy/PublicHealth/41503" target="_blank">class-wide label change put forth by the agency in September</a>, limiting the drugs to patients with severe, refractory pain – not moderate pain.<br />
<strong>Patent Protection at Play?</strong><br />
In a statement to <em>Journal Sentinel/MedPage Today</em>,
drugmaker Zogenix, of San Diego, said the company intends to develop an
abuse-deterrent formulation of the drug and is "committed to advancing
the program as rapidly as possible."<br />
But during the FDA advisory
committee hearing last December, a company spokesperson said an
abuse-deterrent formulation of the drug is only in pre-clinical
development and would be "several years away from the market."<br />
Regardless
of when the updated version comes to the clinic, the strategy of
releasing one product first, followed by an upgrade, undoubtedly extends
patent-protection on the drug, granting it additional years of
exclusivity.<br />
Twillman said his group is concerned about the lack
of abuse-deterrent properties with Zohydro because it can put patients
at "unnecessary risk if they chose to alter their medication."<br />
"While
we do not believe that every person using long-acting opioids needs to
use tamper-deterrent products, we would prefer to see that all new
opioid pain relievers incorporate these features," Twillman said.<br />
<a href="http://sunnybrook.ca/research/team/member.asp?t=11&m=94&page=172" target="_blank">David Juurlink, MD, PhD</a>,
director of pharmacology and toxicology at the University of Toronto,
said even parties who typically disagree about the role of opioids in
chronic pain "agree that abuse-deterrent products make sense."<br />
"The
FDA agrees too. Yet in Zohydro, they have granted approval to pure,
high-dose hydrocodone in a formulation that is not abuse-deterrent,"
Juurlink said. "If the FDA was really interested in protecting the
public, they would have said, 'No thanks. There's no therapeutic void
here, and we have too many people dying of opioids in this country to
justify approving Zohydro.'"<br />

does not really look the part of World Series hero. Not too tall, kind
of squat, a designated hitter on a team that already has one.<br />
He is a dead pull hitter. There is nothing complicated or secret about his swing.<br />
"He's like the little guy in the pinball game," <a class="taxInlineTagLink" href="http://www.latimes.com/topic/sports/baseball/boston-red-sox-ORSPT000005.topic" id="ORSPT000005" title="Boston Red Sox">Boston Red Sox</a> catcher <a class="taxInlineTagLink" href="http://www.latimes.com/topic/sports/baseball/david-ross-PESPT000010385.topic" id="PESPT000010385" title="David Ross">David Ross</a> said. That is not a bad way to sum up his career. Gomes is a baseball itinerant.<br />

With their starting pitcher gutting out four innings with a sore
right shoulder, their right fielder out with back stiffness, and their
first baseman on the bench because their designated hitter needed a
place to play, Gomes stepped up as the star of a 4-2 victory over the <a class="taxInlineTagLink" href="http://www.latimes.com/topic/sports/baseball/st.-louis-cardinals-ORSPT000027.topic" id="ORSPT000027" title="St. Louis Cardinals">St. Louis Cardinals</a>.<br />
With one swing — and, oh, how he enjoyed it — Gomes lifted the Red
Sox to the unlikely triumph that tied the World Series at two games
apiece and ensured it will return to Boston for a Game 6.<br />
"If I tried to put it into words, I'd probably screw it up," Gomes said.<br />
Gomes is 32. In 2001, he was the 18th-round draft pick of a franchise then known as the <a class="taxInlineTagLink" href="http://www.latimes.com/topic/sports/baseball/tampa-bay-rays-ORSPT000221.topic" id="ORSPT000221" title="Tampa Bay Rays">Tampa Bay Devil Rays</a>.<br />
"There weren't that many people that knew that was a professional team," he said.<br />
He stopped in Cincinnati, Washington, Oakland and Boston in the last
three years alone. He was on the wrong side of 30 before any team
guaranteed him more than $1.75 million for a season.<br />
"Skin of a rhinoceros," Gomes said. "Heart of a lion."<br />
The Red Sox needed a platoon outfielder, so Gomes signed with a
last-place team best known last year for a successful clubhouse mutiny
against its manager.<br />
He blended in famously, he and his beard. When the <a class="taxInlineTagLink" href="http://www.latimes.com/topic/unrest-conflicts-war/terrorism/boston-marathon-bombing-%282013%29-EVCAL00036.topic" id="EVCAL00036" title="Boston Marathon Bombing (2013)">Boston Marathon bombings</a> struck, Gomes swung two bats with the names of four victims on the barrel, then auctioned off the bats for charity.<br />
There was the baseball part. Gomes did what he was supposed to do,
hit whenever he could, play some left field. He hit 13 home runs, four
as a pinch-hitter. He helped turn the clubhouse into a place where
teammates supported each other rather than turned on one another.<br />
That kind of support was what the Red Sox envisioned from Gomes on Sunday. He was not in the original lineup. But <a class="taxInlineTagLink" href="http://www.latimes.com/topic/sports/baseball/shane-victorino-PESPT000010483.topic" id="PESPT000010483" title="Shane Victorino">Shane Victorino</a>'s back tightened up and, all of a sudden, Gomes was batting fifth, right behind David Ortiz.<br />
There was a delay before the start of the sixth inning. As baseball
paused to raise money for cancer research, Gomes held up a sign with the
names of two victims — his high school baseball coach, now deceased,
and Brady Wein, a 5-year-old battling <a class="taxInlineTagLink" href="http://www.latimes.com/topic/health/diseases-illnesses/leukemia-HEDAI0000054.topic" id="HEDAI0000054" title="Leukemia">leukemia</a>.<br />
Then, as the Cardinals warmed up, Ortiz gathered the Boston players and told them to relax and have fun.<br />
The opportunity to play in the World Series, he said, does not come around very often.<br />
"If this guy wants to rally us together for a pep talk, it's like 24 kindergartners looking at their teacher," Gomes said.<br />
This is the first World Series for Gomes. Number of career at-bats
before his first postseason home run, the one he hit in that sixth
inning on Sunday: 2,999.<br />
He paused to admire his majestic shot, then pumped his right arm into
the sky as he rounded first base, and all the way to second, and again
at home plate. He insisted he had not thought in advance about how
flashy his trot might be.<br />
"I don't think I'm that good to plan out my home runs in the World Series," he said.<br />
He was good enough that the Hall of Fame wanted his bat. He was happy
to donate it. The ball landed in the Boston bullpen, so Gomes was lucky
enough to get it back, a souvenir of his moment in the spotlight, 10
years in the making.<br />

WASHINGTON — The Obama administration hired a general contractor to
fix its troubled health insurance website and promised Friday that the
key feature of the president's healthcare law would be running smoothly
by the end of November.<br />
That target date was the
first public deadline officials offered for troubleshooting the
glitch-riddled website. In announcing the time frame, <a class="taxInlineTagLink" href="http://www.latimes.com/topic/economy-business-finance/jeffrey-zients-PEBSL000951.topic" id="PEBSL000951" title="Jeffrey Zients">Jeffrey Zients</a>,
the management consultant enlisted to assess the situation,
acknowledged that dozens of unresolved problems remained, including
software flaws — contradicting administration officials' early claims
that unexpected traffic volume was the main cause of the frequent error
messages, frozen screens and other problems.<br />
Still, Zients said the site would be running smoothly for the "vast majority" of users by the end of next month.<br />
"Healthcare.gov is fixable,"
he told reporters Friday. "It will take a lot of work, and there are a
lot of problems that need to be addressed. But let me be clear:
Healthcare.gov is fixable."<br />
Quality Software Services Inc., a unit of <a class="taxInlineTagLink" href="http://www.latimes.com/topic/economy-business-finance/unitedhealth-group-inc.-ORCRP015915.topic" id="ORCRP015915" title="UnitedHealth Group Inc.">UnitedHealth</a> Group and one of the three major contractors that worked on the site, were chosen to lead the repair process, officials said.<br />
Hiring a private firm for the site's rehabilitation was a tacit
acknowledgment that the government's attempt to manage multiple
contractors and complicated technology had gone awry. During the three
years of design and construction of the online insurance marketplace,
the Centers for <a class="taxInlineTagLink" href="http://www.latimes.com/topic/health/government-health-care/medicare-HEPRG00002.topic" id="HEPRG00002" title="Medicare">Medicare</a> and <a class="taxInlineTagLink" href="http://www.latimes.com/topic/health/government-health-care/medicaid-HEPRG00001.topic" id="HEPRG00001" title="Medicaid">Medicaid</a> Services has acted as the systems integrator, a role some critics have said was outside the agency's expertise.<br />
Since its Oct. 1 rollout, the site has shut out millions of users,
thanks to a registration system that was initially broken as well as to
problems with the application function.<br />
The late November target for repairs is slightly sooner than some had
predicted. Contractors who worked on the site told lawmakers on
Thursday that they believed it could be functioning well by Dec. 15, the
last day consumers can enroll for coverage to begin Jan. 1.<br />
In order to avoid a tax penalty for not carrying insurance, consumers must enroll in a plan by March 31, officials said.<br />
Putting a contractor in charge was unlikely to take much pressure off
the administration in the coming weeks. Several Republicans are calling
for the resignation of Health and Human Services Secretary <a class="taxInlineTagLink" href="http://www.latimes.com/topic/politics/government/kathleen-sebelius-PEPLT007554.topic" id="PEPLT007554" title="Kathleen Sebelius">Kathleen Sebelius</a>, while 10 Democratic senators are urging the administration to give consumers more time to enroll.<br />
On Friday, Republicans threatened to serve Sebelius with a subpoena
if she did not provide documents related to the website's woes.<br />
In a letter sent Thursday, Sen. <a class="taxInlineTagLink" href="http://www.latimes.com/topic/politics/government/lamar-alexander-PEPLT007754.topic" id="PEPLT007754" title="Lamar Alexander">Lamar Alexander</a> (R-Tenn.) and Rep. <a class="taxInlineTagLink" href="http://www.latimes.com/topic/politics/government/darrell-e-issa-PEPLT003178.topic" id="PEPLT003178" title="Darrell E Issa">Darrell Issa</a> (R-Vista) asked for a detailed description of the technical problems and how much the government has spent to address them.<br />
They chastised Sebelius for not providing the documents after an Oct.
10 request, while she has "been a frequent guest on numerous news and
television comedy programs subsequent to Oct. 1, 2013," wrote Alexander,
the ranking member of the Senate Health, Education, Labor and Pensions
Committee, and Issa, chairman of the House Oversight and Government
Reform Committee.<br />
A department spokeswoman said that the lawmakers' request was "extremely broad" and that the recent <a class="taxInlineTagLink" href="http://www.latimes.com/topic/politics/government/u.s.-government-shutdown-%282013%29-EVGAP00081.topic" id="EVGAP00081" title="U.S. Government Shutdown (2013)">government shutdown</a> — a result of House Republicans' effort to delay the health insurance mandate — had slowed the response.<br />
Lawmakers have repeatedly been told "that we intend to accommodate
their interest in better understanding our efforts to implement" the
healthcare law, Health and Human Services spokeswoman Joanne Peters
said.<br />
On Friday, Sebelius said she had been unaware of the extent of the
problems before the site went live. At a visit to a health clinic in
Austin, Texas, she told reporters she "didn't realize it wouldn't be
operating optimally before the launch."<br />

spike in radiation levels at the station after a large earthquake
struck in the ocean east of Japan, triggering a small tsunami.<br />

of damage on land from the quake, classified as magnitude 7.1 by the
Japan Meteorological Agency, which struck about 370 km (230 miles) out
to sea. Earlier the agency said the quake had a magnitude of 6.8.<br />
<span id="midArticle_1"></span>Japanese television said a 30 cm (1 foot) tsunami had reached Japan's east coast.<br />

A spokesman at Tokyo Electric Power Co
(TEPCO), operator of Fukushima, said some workers had been ordered to
evacuate to higher ground after the quake, but that there was no damage
or change in readings at radiation monitoring posts around the plant.<br />

were seen leaving a port south of the Fukushima Daiichi nuclear power
station, where three reactors had meltdowns in 2011 after an earthquake
and tsunami knocked out cooling and power, as a precaution after the
tremor. Buildings shook as far away as Tokyo, 230 km south of the
nuclear plant.<br />

Meteorological Agency issued a "yellow" warning shortly after the 1710
GMT quake that a small tsunami would reach the northeastern Japanese
coast.<br />

when a tsunami is not expected to exceed a height of 1 metre (3 feet),
far smaller than the wave that hit the Fukushima plant in 2011 and
devastated large swathes of Japan's eastern seaboard.<br />

The National Security Agency says an internal system error caused its
website to go down for several hours Friday during a scheduled update.<br />
There had been speculation on the Internet that the online site had
been hit with a denial-of-service attack, but the agency said that was
not true.<br />
The NSA said the problem would be resolved Friday night.<br />
After the website went dark, a Twitter account affiliated with the hacker group Anonymous appeared to claim responsibility.<br />
A tweet from the handle @AnonyOps read, “Aww don’t panic about nsa.gov being down. They have a backup copy of the internet.”<br />
There is no evidence that any information or data has been stolen
from NSA’s network, and it is not clear what caused the outage, The Wall
Street Journal reported.<br />

TOKYO — A magnitude 7.3 earthquake struck offshore of Japan’s
crippled nuclear power plant early Saturday, triggering small tsunamis
but causing no damage.
<br />
An earthquake official with the Japan Meteorological Agency
said the quake was an aftershock of the magnitude 9.0 earthquake and
tsunami that struck the same area in 2011, killing about 19,000 people
and devastating the Fukushima Dai-Ichi nuclear power plant.<br />
<br />
There was no damage and only one minor injury was reported, the
national Fire and Disaster Management Agency said. Tsunamis of up to 40
centimeters (15 inches) hit four areas along the coast, but a tsunami
advisory was lifted less than two hours after the quake.<br />
The
epicenter of the 2:10 a.m. Tokyo time (1710 GMT) quake was about 290
kilometers (170 miles) off Fukushima, and it was felt in Tokyo, 300
miles (480 kilometers) away.<br />
“It was fairly big, and rattled
quite a bit, but nothing fell to the floor or broke. We’ve had quakes of
this magnitude before,” Satoshi Mizuno, an official with the Fukushima
prefectural government’s disaster management department, told The
Associated Press by phone. “Luckily, the quake’s center was very far off
the coast.”<br />
Mizuno said the operator of the troubled Fukushima
plant, Tokyo Electric Power Co., said no damage or abnormalities had
been found.<br />
Japan’s meteorological agency issued a 1-meter
(3-foot) tsunami advisory for a long stretch of Japan’s northeastern
coast. It put the quake’s magnitude at 7.1, while the U.S. Geological
Survey recorded a magnitude of 7.3. The U.S. Pacific Tsunami Warning
Center did not post warnings for the rest of the Pacific.<br />
The
meteorological agency reported tsunamis of 40 centimeters in Kuji city
in Iwate prefecture and Soma city in Fukushima, as well as a
20-centimeter tsunami at Ofunato city in Iwate and a 30-centimeter
tsunami at Ishinomaki in Miyagi prefecture.<br />
All of Japan’s 50
nuclear reactors remain offline as the government decides whether they
meet more stringent requirement enacted after the 2011 quake, which
triggered multiple meltdowns and massive radiation leaks at the
Fukushima plant about 250 kilometers (160 miles) northeast of Tokyo.<br />
A
string of mishaps this year at the Fukushima plant has raised
international concerns about the operator’s ability to tackle the
continuing crisis.<br />
